What to Bring to the Day of your Pre-Surgical Testing Appointment

WHAT TO BRING ON THE DAY OF YOUR PRE-SURGICAL TESTING APPOINTMENT

  • Medications that your child is currently taking
    • You can bring the actual bottles/vials/packages as long as these do not need to be refrigerated
    • Please do not bring individual pills outside of their container
    • You can also bring a detailed list of the medicines (including over the counter) with dosages listed in milligrams (if known)
  • Immunization records
  • Copy of recent blood work (within 6 months) or other recent studies (within one year) that your child has had (X rays, EKGs, ECHOs, EEGs, Sleep studies, Ultrasounds, CT scans, MRIs)
  • Picture identification of yourself( parent/legal guardian)
  • Medical insurance card
  • Copy of court papers designating legal guardianship (if applicable)
  • Written list of questions that you or your child may have
